How to use insurance?

When you become ill or need medical attention due to an accident

Immediately contact the Insurer’s Assistance Center to get help.

Depending on your needs, the Assistance Center provides advice or information about what medical facility the client should go to.

Contact is possible 24 hours a day, 7 days a week by calling

Medical insurance covers only sudden, unexpected illnesses that require immediate medical attention.

Do you make an appointment yourself with prior consent of the Assistance Center? In that case you may submit a claim for a refund only AFTER you have been charged for the appointment. To obtain a refund  you must send the claim form with a set of documents listed below.

In order to get help you’ll need:

Refund cost:

Refund is possible provided that you have obtained consent from the Assistance Center.

What documents should you send?

To obtain a refund of the costs incurred, you must send an application together with a complete set of documents.

When you have completed treatment and want to receive payment due to an accident

  • Inform the Insurer about the incident (after completion of treatment, but no later than 3 years from the occurrence of the accident)
  • Complete the documents
  • Apply for a compensation

What documents should you send?

  • copy of medical documentation

  • certificate of treatment and rehabilitation completion

  • copy of policy or insurance certificate

  • police reports (if there are any)

If the application with the claim is incomplete, information will be sent immediately about the need to complete it.
If the application is complete, a decision will be issued within 30 days from the application receipt date.

In case of an accident resulting in the death of the insured person

  • Complete the documents
  • Apply for compensation

What documents should you send?

  • medical documentation that certifies the direct cause of death

  • a prosecutor’s office report (if applicable)

  • autopsy results (if performed)

  • declaration of a new beneficiary appointment

  • a copy of the marriage certificate or a copy of birth certificate (if the beneficiary is the offspring or a spouse of the insured person)

  • a copy of the beneficiary’s ID card certified by a notary

  • the original death certificate or a notarized copy
